And if they don't eat put a little health check to stay awake or be forced
to sleep so the body can recover from that undue amount of extra work it
just did. ( And no that HEA check wouldn't be instantaneous more like an
hour later roll 1d6, 2 hours 2d6... ). 

I liked Jevan's example of when you get Mutated for Exceptional STR your
muscles reposition themselves for better leverage and from that point on
you cannot move/stand like you used to plus you can't fit normal armor.
Consequences. You freak. :)
