Rulings and rule changes

One thing to keep in mind when discussing what was decided on this list
and in GM meetings:

The process we came up with was to decide on what rulings or rule changes
we wanted, then to submit them to Dan for final approval. If Dan approves, 
then he would let us know, and prepare an errata/addendum for our rulebooks.

To my knowledge, very few of the topics we discussed were ever submitted to
Dan. We should keep this in mind when we're figuring out what was 'passed'
and what wasn't.
